Melton City Council – Community Care

Supporting people with a disability, carers and seniors

Community Care is dedicated to ensuring that our community is an inclusive and accessible place for all. Council supports seniors, people with disability and carers through a range of services and activities. Council’s stall at the CARE Melton Expo will provide an opportunity for you to access information, speak with Council officers and ask any questions that you may have.

Help at Home
Council offers a range of home-based services to eligible residents whose capacity for independent living is at risk, due to frailty in older age, living with disability or caring for an individual.

Domestic Assistance

Assistance with general housekeeping tasks to maintain cleanliness and safety, unaccompanied shopping or bill paying.

Personal Care

Assistance with showering, toileting, bathing, grooming, dressing and other non-nursing tasks.

Respite

Support in the home or the community so carers can take a break.

Social Support Individual

Accompanied assistance with grocery shopping or transportation to medical appointments.

Property Maintenance

Assistance with minor household repairs and modifications for safety and security. Services may include installation of held hand showers,  rails, smoke detectors etc.

Food Services

Provide delivered and centre based nutritious three course meals.

Social Support Programs and Activities
Encourages and promotes social connections, independence and well-being, whilst providing respite and support to carers. Some of these activities may include gentle exercises, games, special interest groups and outings to help maintain health, flexibility, mobility and self esteem in a supportive environment that maximises individual wellbeing.

Accessing services
Before you are able to access services, a A referral for an assessment will be required.

  • For people under 65 living with a permanent disability please contact Council’s Home and Community Care Program for Younger People Assessment Officer on 0499 303 581 or email haccpypintake@melton.vic.gov.au.
  • For frail, older people 65 years or older (50 years or older and identify as an Aboriginal or Torres Strait Islander person) who need assistance with daily living to remain living independently at home and in the community, contact My Aged Care on 1800 200 422.

Other Services and Activities
Community Care also offers:

  • Melton and Taylors Hill Men’s Sheds

A great place for men to meet, learn or share skills, or just come along for a cuppa and a chat.

  • A wide range of exercise programs delivered across the municipality.
  • ‘Access Keys’ for both Melton and Caroline Springs Libraries. Access Keys are guides which offer a level of predictability for people with a disability. The Access Key contains colour photos, supportive text and sensory guides.
  • Three Changing Places Facilities at the Melton Library and Learning Hub, Melton Waves Leisure Centre and the newly built Cobblebank Stadium.
